Minnesota Aging Pathways: From Medicare to Preadmission Screening

Minnesota Aging Pathways (formerly Senior LinkAge Line) provides comprehensive help to older adults in Minnesota via phone, chat and email. Aging Pathways helps with Medicare and long-term care; it helps older adults remain in their homes and offers caregiver supports; and it locates all types of resources. This presentation will provide an overview of the MAP work as well as specific programmatic information.
